JOB SUMMARY
An Outreach Worker connects vulnerable populations with essential services. They identify needs, provide information, make referrals, develop programs, collaborate with other professionals, maintain records, advocate for clients, offer crisis intervention, and conduct educational workshops.
ESSENTIAL DUTIES AND RESPONSIILITIES
Outreach activities toward community members, focusing on targeting youth, young adults, family members, and key support systems. Required outreach activities include:
- In-person (e.g., tabling at local events) and digital (i.e., virtual, phone, and text) strategies for reaching and engaging target groups.
- Provide wrap-around support (e.g., emergency assistance for youths, young adults, and families who are ineligible for public benefits).
- Establishing partnerships with local community-based organizations to provide public education about available services.
Wellness activities leveraging trained paraprofessionals working continuously in their communities to support mental wellness. Wellness activities include:
- Group facilitation, providing psychoeducation on a variety of topics that support and promote mental wellness.
- Participate in public education on mental wellness (community level and individually focused).
- Facilitate organized recreational activities that promote community wellness (e.g., fun runs/walks, creative activities such as arts/crafts and music, or other organized activities).
Assessment, referral, and linkage activities. Required assessment and referral activities include:
- Screen individuals for common mental illness symptoms using OMH approved instrument(s) and refer them to known public mental health clinical services as appropriate.
- Maintain accurate, current lists and directories for use in making referrals and linking to relevant programs, services, and support.
- Complete referrals and linkages through warm call transfer, personal accompaniment or in-person visits, or virtual introductions of those with expressed or assessed need.
Treatment-adjacent service offerings. Required offerings include:
- Brief individual and group counseling to address intense stress, trauma, and/or anxiety.
- Family education and counseling.
- Academic and vocational support.
Data Reporting and Meetings
- Complete required daily and weekly data reports through RedCap
- Attend daily team huddles
- Participate in Learning Collaborative Trainings with OMH
- Participate in weekly supervision with Team Leader
- Other duties, as needed
- Work evenings or weekends, as needed

QUALIFICATION
- High school graduate, some college, or college graduate
- Relevant work experience
- Experience with client engagement and motivational interviewing
- Proficient in the following applications: Office 365 (Word, Excel, PowerPoint), Forms, Teams, and Zoom
- Ability to handle multiple tasks and work independently
- Excellent oral, written, and interpersonal communication skills
- Ability to deal effectively with clients, staff, and third parties and maintain confidentiality
