Job Summary

We are looking for a Case Planner to join our team and help provide support to referred families and children. The ideal candidate will have a strong background in social work, experience with providing services in the home, and providing support services to children and families. The Case Planner will be responsible for assessing the needs of families, developing action plans in accordance with the program Family Connections model, and ensuring that all goals are met. This position requires excellent communication skills, an ability to work independently, and a commitment to providing quality service.

PRINCIPLE RESPONSIBILITIES & DUTIES:

• Capacity to engage families in prevention, home based services.
• Conduct evidence-based assessments to determine the strengths and needs of clients.
• Develop and implement treatment plans with the family.
• Collaborate with other professionals to coordinate services for clients.
• Provide advocacy and support to clients in order to ensure their safety and well-being.
• Maintain timely, accurate records of client information and progress.
• Monitor client progress and make necessary adjustments to the case plan as needed.
• Assist clients in developing life skills and self-sufficiency.
