Case Aide/Caseworker

JOB SUMMARY

The Case Planner is responsible for managing and coordinating services for individuals or families involved in social services programs. This role involves conducting assessments, developing case plans, providing referrals to support services, and monitoring progress towards goals. The Case Planner collaborates with clients, service providers, and other professionals to ensure comprehensive support and advocacy for clients in need. Strong communication, organizational, and empathy skills are essential for success in this role.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Supports Case Planners with their assigned families including home visits.
  • Supports new Case Planners by joining them on visits to families.
  • Finds resources for families and helps families access these resources.
  • Completes school visits for children in program and supports families with obtaining vital services (medicals, food pantries, public assistance, apartment issues, etc.)
  • When required by CP unavailability (due to illness, vacation), assists Case Planners in covering families by making home visits to assess and ensure Family Safety and Risk
  • Participates in FTC and ACS meetings.
  • Documents services provided to families in the NYS Data base system (CONNECTIONS) within required timeframes.
  • Documents contact with families in the ACS data base system Preventive Organization Management Information System (PROMIS) within timeframes.
  • Effectively uses ACS Workforce and other trainings to improve skills needed by a CA.

Qualifications

  • BA in Human Services
  • Prior experience with children and families
  • Bi-lingual Spanish is optimal to meet needs of some program families.
  • Ability to engage children and families with prior/current ACS involvement including those adverse to ACS services.
  • Provides services to families within their home and community.
  • Knowledgeable about community resources or capacity to research and contact community resources.
  • Capacity to assess child safety and risk factors.
  • Knowledge of CONNECTIONS and PROMIS data base systems or ability to learn these programs.
  • Good communication skills both written and verbal.
  • Capacity to complete all documentation within required timeframes
  • Cultural Competence/Cultural Humility
  • Relates well to families, staff and supervisors.
